What to Bring to the Office
For first visit: Bring your insurance card (rather than just an image of your card on your smart phone so we can scan it), your photo ID, copies of your most recent medical tests and older records for long standing problems.
For thyroid issues, bring pre-treatment blood tests, especially TSH if available. For thyroid nodules, bring scans, ultrasounds, biopsy, and surgery records if available.
For patients with diabetes, bring your glucose meter to download if you have one, and your most recent blood tests, including Hemoglobin A1c, creatinine, and cholesterol/lipid results. If you have a continuous glucose monitor or a finger stick blood sugar meter that has data on the internet, make sure the most recent data is uploaded. We prefer if you can print out data summaries; we are especially interested in "modal day" or "standard day" downloads with real data points, or tracings of continuous glucose monitor results with individual day glucose plots.
For patients with osteoporosis, bring old bone density study results if available.
